The US military has announced that it attacked a Doctors Without Borders (MSF) clinic in Afghanistan because of "human error."
An investigation ruled that the AC-130 gunship crew confused the Kunduz clinic with a nearby Taliban building. Those deemed responsible have been suspended.
The attack left at least 30 civilians dead and has been called a war crime by MSF.
