The deadly attack on a hospital in the Afghan city of Kunduz, in which 22 people were killed, was a mistake happened as a result of an error in the chain of command, says General John Campbell, the commander of the US forces.
"We would never intentionally target a protected medical facility," said General Campbell.
The hospital was run by Doctors without Borders, and the organization's workers made up a large portion of the casualties.
