Former Mossad spy Ben Zygier, aka "Prisoner X," hanged himself with a bedsheet in his prison cell, according to an Israel Prisons Services (IPS) inquiry report quoted in Maariv Tuesday.

According to the newspaper, the inquiry took place in December, 2010, shortly after Zygier's death.

It found that Zygier had planned to end his life, when he was incarcerated in solitary confinement in Block 15 of the Ayalon Prison.

After waking up from his night's sleep and receiving his breakfast, Zygier took the bedsheet that covered his bed and took it to the bathroom cell. This action is apparently normal, if a prisoner wants to wash his bedsheet. However, he then ties the bedsheet to the bars in the bathroom's small window.

He apparently made use of a small stool as well, in hanging himself.

Maariv added that because Zygier was not considered a suicide risk, his guards did not check up on him every few minutes, but only every 20-25 minutes, and watched him with the security cameras the rest of the time. This was enough time for him to carry out the suicide.

The IPS inquiry report, as well as the operations log and the forensic findings of the police's forensic lab were handed over to a judge, Dafna Blatman-Kardai, who wrote the official report on Zygier's death.

The IPS told Maariv that there is no way to prevent a prisoner's suicide unless he is diagnosed by health professionals as having suicidal tendencies, in which case he is placed in a special padded cell, or shackled to his bed 24 hours a day.
