I just noted it was missing and that my opinion is that in glossing over it, a mistake was made.  Maybe Monsey people are reflexively extreme?\n\nAs for problems of \"karet\", you could of course assume the position of the Ravad who permits entrance to all areas of the Har Habayit as he holds there is no \'karet\'.  Or the position of other Rishonim that the sanctity no longer applies or that without walls there is no sanctity.  In other words, there are many lenient opinions.  Historically, there was a Bet Knesset near the Cotton Gate until after the Crusader period.  Even the Rambam ascended. In fact, the Temple Mount appears prominently in the center of their map.
